Report of accidents

A report of the accident should be sent to the Controller and the Sub-Divisional Officer (SDO), as soon as possible, with full details, through telephone or any other means.

As per under Section 33 (1) of the Indian Electricity Act, 1910, a report of the accident should be prepare by the concerning Sub-Divisional Officer (SDO) according to instructions given in State Order No. 123-E1 / 10-E-1-1927, dated 9th July 1928 and should be submitted to the Electrical Inspector of the Uttar Pradesh Government as well as the copies should be also sent to other officers about whom the Instructions have been given in the state order. These instructions are printed on the other side of the accident form. Officials should prepare this report in their own handwriting and also submit to the concerned Chief Engineer.

After the accident,as soon as possible, the concerned Assistant Engineer and the Executive Engineer will conduct a departmental investigation at the accident site and report the results of the inquiry, on the prescribed form, by the proper way, to the Chief Engineer.
