Documents for Family Pension/Death Gratuity

The applicant for family pension has to submit following forms/ documents for family pension/death gratuity:

1 Pension form part-2 Three copies
2 Pension form part-3 Three copies
3 Certified copy of death certificate issued by Municipality officials Three copies
4 No dues certificate Four copies in original form
5 Certified copy of successor certificate Three copies
6 Letter of Indemnity certified by notary (family affidavit with the consent of all dependents that family pension/death gratuity and other amount is to be paid to the applicant and other members) Three certified copies along with original.
7 Letter of Indemnity no-dues certificate (attachment-II) Three certified copies along with original
8  For last withdrawal of GPF the required form "C", and for amount of related insurance scheme  form "G-1" , in three copies each (along with page no 29 to 34, other required certificates, certified copies of family affidavit, last available accounts details of GPF)  
Note In case of such employee whose pension is to be sanctioned by Corporation Level / Chief Engineer (Hydel), the above form should be submitted  in four copies so that the above details can be sent to the corporation / Chief Engineer for the advance action of sanction  of pension etc.

Special attention is desired on these precautions while filling up the pension form

  • Applicant must write his/her name in full and clearly.
  • After carefully reading the declaration mentioned in the section of the form, please sign before the evidence.
  • As far as possible, the signature of witnesses (evidence) should be of those employees where the applicant was working at the time of retirement.
  • In case of inability to sign, the impressions of left thumbprint and fingerprints in case of men, or right thumbprint and fingerprints in case of women should be taken, and this according to above point number 3.
  • Identification marks should be taken for such parts of the body such as face, hand that is visible. If there is no such mark in the visible parts, the marks of the inner part can also be mentioned.
  • Generally the Board employee should get three joint photos along with his wife/husband pasted separately in column 6 of pension form (part-3) and get it certified, and one certified copy should be submitted separately in an envelope to be pasted on the disbursement half. If either of husband/wife is alive, or the husband/wife are separated, then only the family pensioner will paste his photo. The applicant for family pension has to paste only his photograph.
  • Write 'zero' in the column in which there is nothing to fill.

Note:

  • If there is any difficulty or query in filling the forms, please contact the circle office on any working day so that the pension form could be filled correctly and there should be no unnecessary delay in settling claims.
  • For last withdrawal of GPF, the applicant must fill up the required form "C", and "G-1" in three copies each along with other required certificates, such as certified copies of death certificate, succession certificate, family affidavit, along with last available accounts details of GPF (GPF slips).
