Forms/records/information to be filled up by the circle office from where retirement is taking place/death has occurred during service (R-II and D-II)

1 Pension form part-4 Three copies
2 Pension form part-4 Three copies
3 Pension form part-5 (A) Three copies
4 Certificate related to Corporation/Nigam residence Three copies
5 Departmental no dues certificate Three copies
6 L.C.P. Three copies
7 Service book (after verification of complete entries) In original.
8 Declaration and no-dues certificate in case of building / car / scooter and other advance from Corporation / Nigam related Three copies (certified)
9 Certified copy of office memorandum in case of provisional pension/provisional gratuity. Three copies
10 Office memorandum and details regarding payment of encashment of balance earned leave at the time of retirement. Three copies
11 To be sent after the date of retirement on the basis of last payable certificate (pension form-2) ----
12 Other documents which are necessary for the case as per check list.  

Note:-

  • Write zero in the column in which there is nothing to fill in pension form.
  • A format is enclosed for check service book. Writing the entries and the page number of the service book will clear the facts of the service book and also help in completing the entries as per rules and attesting the same if any shortcoming is noticed.
  • The service book along with pension form should be sent after inquiry by the concerned regional accounts officer.
  • As soon as the required form "A" or "B" is received from the applicant for final GPF payment, the complete details of GPF along with necessary information and inquiry letter should be sent to concerned accounts officer.
  • Every page of the pension forms must have the verified signature of the head of the office / competent officer and all documents being attached should be certified and have clear forwarding date.
